Is it Actually a Prostate Issue?

Finding Relief for Men’s Pelvic Pain in El Cerrito, CA

If you’re struggling with chronic pelvic pain, testicular ache, sudden urinary urgency, or the feeling of sitting on a golf ball, someone has probably diagnosed you with chronic prostatitis or chronic pelvic pain syndrome (CPPS), maybe even pudendal neuralgia.

The first plan of attack from the urologist is usually multiple rounds of antibiotics. These sometimes help short term, but if your symptoms still haven't cleared up, the cause of your pain might not be an infection at all. Surprised?

For many people, these symptoms can actually be caused by tight or overactive muscles in the pelvic floor, meaning the deep hammock of muscles supporting your bladder, bowel, and sexual function are stuck in a chronic state of tension or guarding. There is also usually a nervous system or neuroplastic component because the brain doesn’t always interpret tension as pain.

Your Tight Pelvic Floor Doesn’t Listen to Reason

Chronic Pelvic Pain Syndrome (CPPS) and chronic prostatitis are incredibly common, yet rarely discussed. When pelvic floor muscles remain clenched and guarded, they can mimic the symptoms of a true prostate issue, like:

  • Pain or burning during or after urination

  • Urinary frequency or urgency

  • Having to get up multiple times overnight to pee

  • A vague or deep aching or pressure in the perineum (the space between the scrotum and anus). Aka ‘taint, ABC (ass ball connection), gooch, etc.

  • Discomfort that worsens after sitting for long periods

  • Erectile changes

  • Painful ejaculation

  • Hard flaccid syndrome

  • Low back pain or discomfort

Looking for Men’s Pelvic Floor Physical Therapy in the East Bay?

When urologists or other doctors finally recognize that pelvic tension is possibly contributing, they typically refer patients to men's pelvic floor physical therapy or a specialized men's pelvic PT in the East Bay Area.

At Soft Power Pelvic Health, our specialized pelvic floor occupational therapy (OT) lens utilizes similar evaluation tools and orthopedic manual therapies, myofascial trigger point release (if you believe trigger points exist), and biofeedback therapy to treat chronic pelvic pain.

As an OT, Dr. Rebs expands on structural treatment by looking at your whole life. We evaluate how chronic stress, nervous system dysregulation, sleep habits, and sitting routines wind up your pelvic floor tension. We don’t just address the anatomy; we give you attainable strategies to untangle long term pain patterns from your daily routines. Dr. Rebs also uses Pain Reprocessing Therapy (PRT) to address possible nervous system sensitization and help your brain recalibrate how it’s interpreting sensations of tension.
